Short Term Goal


This brochure will enable the patient to have information about the coronary disease. They will be able to read and understand the causes and some of the prevention tips as advice by medical doctors who serve in this field. Furthermore, this brochure will assist the patient to practice basic life skills that will enable those who are already infected to take reasonable actions against the infection. Another goal  of the brochure will to assist  those who are still healthy to practice best life skills  to prevent themselves from getting infected .Scientists and doctors have also summarized their ideas and utterances about coronary  infection , thus the brochure will enable patients to acquire  outlined reviews stated by medical practitioner  concerning in a brief and understandable design.

Target Audience This brochures does not aim at targeting a specific category of individuals rather it will target the healthy and patients who are already undergoing this infection.

Objective One Objective: Following the brochure, the individual will thus be proficient in understanding all the factors that would reduce this infection for example healthy lifestyle to practice (Khera, 2016).


Type of Prevention: Primary


Rationale: Experiencing how individuals distinguish dissimilar activities that may result into the infection prove understanding to the patient .This is analysis which is one of the domains of cognitive learning (Aliakbari,2015)

cognitive The brochure will have information on factors that could be moderated to control or prevent coronary heart infection. The note will be engraved in the segment of the brochures detailed “keeping the heart healthy”. For this case, the teach-back method will be initiated.

After reading the brochure, different healthy lifestyles skills will be determined by the patient by explain them according to their understanding (Khera, 2016). This evaluation technique has been rated highly for effective cognitive evaluation.

Objective Two. Objective: This brochure will enable an individual to choose healthy foods for instance by avoiding foodstuff such as beef which contain high fat percentage and engage in  taking food rich in fiber for instance fruits and vegez (Centrella-Nigro,2017).

Rationale: Synthesis of information is considered another cognitive approach. It enables the individual to understand different concepts deeper and apply their meaning where appropriate for instance reducing alcohol consumption and avoiding smoking while discerning the substantiality of allowing healthy food into the body (Centrella-Nigro, 2017).

cognitive The brochure will contain a section titled “control intake into the body” This section will ask the individual to have control on what they take and avoid harmful intakes such as alcohol and maintain fruit ,vegetable and nuts for oil. The teach-back technique will be helpful for this objective. It will evaluate the understanding the individual will have acquired about the dangers of alcohol consumption by allowing the individual to write short notes about taking harmful products that may trigger this infection and write notes about the importance of fruits and natural oils as stated in the brochure (Centrella-Nigro, 2017).
Objective Three Objective: This brochure will provide the affiliation between cholesterol and coronary heart infection (Aliakbari, 2015).


Type of Prevention: primary


Rationale: Allowing the individual to outline some of the dangers of cholesterol as explained primarily by the brochure (Graudal, 2017). For instance clogging on the walls of the arteries thus minimizing blood flow into the earth.

cognitive The last section of the brochure titled “cholesterol effects” will explain to the individual how heavy accumulation of this product on the walls of the artery stirs the infection. The fats accumulate on the walls of the arteries restricting entry of blood into the heart.

Another section titled “Avoiding cholesterol” would have details on products which contain cholesterol like high sugar, salt and fat intake (Graudal, 2017).

The teach-back technique will be used by the nurse on the individual after they read the brochure. This will be done by allowing the individual to mark correct choices concerning cholesterol intake. Choices will be given to test the individual’s understanding on food substances which contain high cholesterol. The nurse will then identify the knowledge of the individual by checking if effective choices are made.
